How to add an opt-in form's response page with PDF report to the website?

Now the next step is to link this PDF report page to as opt-in form response page.

So you already know how to add/edit content page. Go to the content page, where you want to add an opt-in form for the PDF report.

Then scroll down to the Optin form.
Choose Yes from Show Optin Form.

Then select your response page (PDF report) in Where should your visitor be sent after opt-in?
